Child Care Providers: Kindergarten Transition Survey (This is a Pilot Project)
Do you provide Child Care or Preschool to a child entering Kindergarten in the fall? The local school districts would like to hear from you about this child's development and skills. This information will help the schools when then they are placing children and will help provide insight for their Kindergarten teacher.
*Make sure you have permission from the child's parent/guardian before sharing any information*

La Grande School District
There are 3 elementary schools in La Grande: Central Elementary, Greenwood Elementary and Island City Elementary.
Online only option: this is an secure, online form that is completed for each child.
Milton-Freewater School District
Online Option: this is a secure, online form that is completed for each child.
Paper Option:
If you would like to complete the survey on paper, download the PDF to complete. You can then mail the survey(s) to the school or call to set up a time for them to pick up the survey(s) from you.
